Patient registration, onboarding and coordination of care will comprise at least 50% of the daily tasks for the referral coordinatorPerforms the following tasks in a correct and timely fashion under the supervision of a pharmacist:
Coordinates the onboarding of new patients to One Infusion Pharmacy by communicating with case managers, nursing and the caregivers to get the appropriate information. Fulfillment Technicians are responsible for coordinating care with external and internal departments to ensure accuracy and completeness of all pertinent clinical, billing and demographic information in accordance with the policies and procedures of One Infusion Pharmacy, state regulations, and other regulatory entities such as Accreditation Counsel for Home Care (ACHC).
