The physician independently provides comprehensive medical care to Veterans residing in the Community Living Center and is responsible for performing admission history and physical examinations, comprehensive assessments, diagnosis and development of management plans, referrals as deemed necessary, inpatient and outpatient consultations, supervision of Geriatric fellows, residents, medical students, and other rotating medical trainees, evaluating referrals for inpatient admission or consultation, leading and participating in family conferences, and serving as an active member of the interdisciplinary treatment team. The physician is responsible for the ongoing management of acute and chronic medical conditions, medication management, coordination of specialty consultations, advance care planning and goals-of-care discussions, discharge planning, and coordination of care to ensure high-quality, patient-centered, evidence-based geriatric care.
