This position is designed as a longitudinal transplant-focused APP role integrated across the continuum of advanced lung disease and lung transplant care, including pre-transplant evaluation, inpatient non-ICU management, outpatient longitudinal follow-up, immunosuppression surveillance, multidisciplinary coordination, and post-transplant continuity of care.
