Cultivates and develops relationships in assigned hospitals, nursing homes, assisted living facilities, physician offices, health centers and the community at large to drive HopeHealth's hospice and palliative care growth in assigned territory. The Care Navigator RN is a subject matter expert in all services offered by HopeHealth including hospice, palliative care, bereavement support, caregiver support, dementia services and community education.
