You may begin the day rounding at a memory care community, meet with a family concerned about a change in a patient’s behavior, coordinate with facility staff regarding a medication adjustment, complete follow-up visits at an assisted living community, and conduct telehealth or care-coordination work later in the day. You will manage the full spectrum of geriatric primary care, including chronic disease, preventive care, acute concerns, medication management, cognitive decline, changes in function, and transitions between healthcare settings.
