Duties and Responsibilities:Complete comprehensive family-based assessments for identified youth, including the Family Wellbeing Assessment and the Strengths and Difficulties Questionnaire;Provide family-centered case management services, and other services relative to program goals and objectives agreed upon with the client based on the Assessment;Develop strength-based, individualized family treatment plans including goals and objectives;Conduct home visits, school visits, and field visits, in addition to office visits, to best meet the needs of the youth and family;Assist in the planning, coordination, development, monitoring, and evaluation of services for youth and families with emphasis on quality of care, continuity of services, and cost-effectiveness;Maintain case records and client files in the Efforts to Outcomes (ETO) database as required by SBCS and its funding sources;Facilitate weekly evidence-based, trauma-informed center-based groups for youth and/or caregivers;Outreach to target population to effectively reach those in need of services;Participate in special projects and activities as related to the agency's goals and objectives;Participate in agency special events, such as fundraisers, annual meetings, and open houses;Participate and assist in implementing program components;Conduct crisis intervention services, and provide referrals to telephone and walk-ins;Participate in Multi-Disciplinary Team meetings with other team members;Coordinate with contacts on other public and private organizations and individuals, as necessary to effectively serve clients.
