The PHN conducts assessment, education, case management, care coordination, immunization, communicable disease services (including TB), and surveillance activities; documents care in the electronic medical record (EMR); and collaborates with interdisciplinary partners to advance equitable health outcomes. Deliver TB services: case finding, medication coordination, therapy adherence monitoring, patient education, provider communication, and collaboration with TB epidemiology; oversee Community Health Worker activities for directly observed therapy.
