The case manager’s role is to provide the necessary ongoing pro-active supports in the continuum of care for assigned clients in the community, and to assist clients in remaining out of the hospitals, jails, other institutionalized care, to decrease homelessness of the clients, and support clients in maintaining independence and community functioning.
