All candidates must meet at least one of the following criteria: Certificate of training or graduation from an accredited MA, Home Health Aide, PCA, CNA, dental or paramedic program or college degree in a related field such as community health, infectious disease, public health or health education and 6 months of relevant experience. Primary responsibilities include supporting clinical workflows, assisting with patient inquiries, coordinating follow-up care and appointments, managing documentation, and facilitating communication across multidisciplinary teams.
