As a PRN RN Case Manager, you will oversee the care of assigned hospice patients, conduct comprehensive assessments, coordinate services, manage symptoms, and support patients and families throughout the hospice journey. Coordinate patient care with physicians, the Medical Director, social workers, chaplains, hospice aides, facility staff, pharmacies, medical equipment providers, and other interdisciplinary team members.
