This full-time role is based in our bustling healthcare center, working directly with patients and their caregivers in a variety of settings including the clinic, adult day centers, patients' homes, and more. Your responsibilities will include conducting face-to-face assessments, delivering and documenting nursing interventions, coordinating all aspects of care delivery, educating participants and caregivers, and assisting with wound care and other procedures.
