Our care portfolio includes community hospitals, academic hospitals, primary and specialty care clinics, senior facilities, facilitated living centers, rehabilitation centers, home health care services, counseling, pharmacies and benefit management services. Some specific duties include pre-admission phone calls, admissions day of surgery, monitoring and continuously assessing patients following procedure until ready for transfer to the floor-as well as Phase 2 care to include intensive patient education that assures readiness for discharge to home.
