The Health Information Coordinator is responsible for maintaining the health information services for the community Skilled Nursing Center in accordance with Federal, State and local laws and regulations along with professional practice standards. RESPONSIBILITIES & QUALIFICATIONS:

The unique responsibilities for this role include but are not limited to the essential functions listed as follows:</p><h3 style="font-size:11pt;font-weight:bold">Health Information</h3><ul><li style="font-size:10pt">Maintains health information services for the community Skilled Nursing Center in accordance with Federal, State/Provincial, local laws and regulations and professional practice standards.</li><li style="font-size:10pt">Reviews and maintains current census.</li><li style="font-size:10pt">Maintains filing system for in-house and discharged guests/residents to facilitate record retrieval and access.</li><li style="font-size:10pt">Assembles, audits, completes and files guest’s/resident’s discharge records within thirty (30) days.</li><li style="font-size:10pt">Maintains system for the timely completion of all charts.</li><li style="font-size:10pt">Audits in-house records for accuracy and completeness according to the established schedule.</li><li style="font-size:10pt">Thins in-house records monthly and promptly file the thinning in the guest’s/resident’s in-house overflow file in an organized manner.</li><li style="font-size:10pt">Maintains current guest/resident information in the computer file and generates the necessary reports in accordance with the established schedule.</li><li style="font-size:10pt">Prepares admission records and chart for the newly admitted guests/residents.</li><li style="font-size:10pt">Retrieves and files the guest’s/resident’s health information records and reports timely.</li><li style="font-size:10pt">Assists the clinical team members in preparing discharge and transfer records.</li><li style="font-size:10pt">Prepares health information records with documented Administrator’s approval, for subpoenas and correspondence in accordance with the established procedures.</li><li style="font-size:10pt">Ensures all health information records are protected and kept confidential in accordance with current laws and regulations.</li><li style="font-size:10pt">Protects all health records from loss, damage or destruction prior to the statute of limitations.</li><li style="font-size:10pt">Assists with Quality Assurance and other committee meetings when instructed by the Director of Nursing.</li><li style="font-size:10pt">Assists in Quality Improvement studies.</li><li style="font-size:10pt">Attends inter-departmental meetings as directed.</li><li style="font-size:10pt">Participates in ongoing in-service training as related to health information, resident rights and other related subjects.</li></ul><h3 style="font-size:11pt;font-weight:bold">Guest/Resident Relations</h3><ul><li style="font-size:10pt">Responds to guest/resident and/or family request in a timely and friendly manner.</li><li style="font-size:10pt">Maintains confidentiality with all resident information.</li><li style="font-size:10pt">Provides excellent customer service to our guests/residents and visitors at all times.</li><li style="font-size:10pt">Ensures the Sunrise Principles of Service are extended to all guests/residents.</li></ul><h3 style="font-size:11pt;font-weight:bold">Risk Management and General Safety</h3><ul><li style="font-size:10pt">Partners with community team to ensure community is in compliance with national/provincial regulations pertaining to occupational health and safety requirements and promotion of Risk Management programs and policies;
