You will support seamless transitions of care by conducting effective sales calls, strengthening relationships, educating partners and families, and coordinating timely referrals. Every encounter reflects our values—trustworthy, capable, compassionate, proactive, and called.Essential FunctionsReferral Development & Sales Execution<ul><li>Serve as a liaison between referral sources, patients and caregivers, and the clinical team to ensure seamless coordination through the care transition process.</li><li>Execute effective, structured sales calls using pre-call planning, rapport building, probing questions, objection handling, and closing techniques.</li><li>Identify and meet the needs of referral partners through consistent communication and service excellence.</li><li>Successfully execute weekly, monthly, and quarterly strategies to increase market share and drive referral growth.</li><li>Initiate, manage, and document consistent sales activities across multiple contacts within each referral source.</li><li>Achieve established referral and admission goals and contribute to overall organizational growth.</li></ul>Relationship Building & Collaboration<ul><li>Build and maintain collaborative working relationships with referral partners, community stakeholders, and internal teams.</li><li>Represent the organization professionally and tactfully across a variety of healthcare settings.</li><li>Work in close partnership with clinical and operational leaders to support timely admissions and appropriate patient onboarding.</li><li>Participate in community outreach, education, and events to increase awareness of services.</li></ul>Care Coordination Support<ul><li>Engage patients, caregivers, and hospital care teams to facilitate appropriate, timely home health or hospice transitions.</li><li>Communicate key patient needs, risks, and updates to internal teams to support safe care delivery.</li><li>Maintain accurate documentation in CRM and EMR systems to support communication and continuity of care.</li></ul>Professional Standards<ul><li>Demonstrate strong time management, organization, and follow-through to meet expectations.</li><li>Maintain accountability to regulatory requirements, agency policies, and compliance standards.</li><li>Participate in required meetings, in-services, and professional development activities.</li><li>Perform additional duties as assigned.</li></ul>Qualifications<ul><li>Bachelor's degree in Healthcare, Business, Marketing, or a related field preferred; equivalent experience accepted.</li><li>Minimum two years of healthcare or related sales experience; hospice or home health experience preferred.</li><li>Demonstrated ability to interact professionally with individuals at all levels, both internally and externally.</li><li>Strong communication, relationship-building, and decision-making skills.</li><li>Proficient with Microsoft Office (Word, Excel, Teams, Outlook) and CRM systems.</li><li>Ability to work independently, prioritize competing demands, and manage a dynamic schedule.</li><li>Reliable transportation with current auto liability insurance.</li></ul>Environmental / Working Conditions<ul><li>Works primarily in the field conducting visits with referral sources across hospitals, skilled nursing facilities, physician offices, and other community settings.</li><li>Requires local travel within the service area; occasional adverse weather conditions may be encountered.</li><li>Flexible schedule required to meet the needs of referral partners.</li><li>May occasionally participate in patient or family meetings to support education and coordination.</li></ul>

Job Summary:The Referral Coordinator is responsible for efficiently managing referrals from outside providers to ensure proper authorization and payments for services. About the Practice:The physicians of Heaton Eye Associates have been serving East Texas’ vision needs for over 40 years. We offer state-of-the-art technology with special services ranging from primary eye care to world-class laser surgery. Our physicians were the first to offer LASIK laser vision correction in East Texas and have performed tens of thousands of procedures since its inception. Heaton Eye offers a wide range of premium lenses including Symfony Extended Depth of Focus lenses, PanOptix® lenses, and Vivity® lenses to offer freedom from glasses and contacts after cataract surgery. Our surgeons were the first in East Texas to offer the RxSight Light Adjustable Lens, the only lens implant that allows patients to preview and adjust their vision after cataract surgery. We are the first and only practice in the area offering EVO ICL, another exciting option offering patients freedom from contacts and glasses. Additionally, we have the only fellowship-trained Pediatric Specialist as well as the only Oculoplastic Surgeon offering cosmetic, medical, and reconstructive surgery as well as state-of-the-art aesthetic services in the area. Athens, whose tagline is Hamburgers-Heritage-Texas, has the charm of a small, southern town while still retaining many qualities of a larger city and is home to the East Texas Arboretum and Botanical Society.Experience:NextGen experience preferredInsurance Verification: 2 years’ experienceExperience processing/managing referrals: 2 years’ experience, demonstrating production and accuracyCertification/Licensure:None Required  Education:High School Diploma or equivalentJob Duties Include:  <ul><li>Receives referral requests from providers and Health Plans, as needed.</li><li>Verifies member’s eligibility and benefits with timely notification to designated staff of eligibility issues.</li><li>Inputs all requests for services received via fax or phone into the system accurately for electronically generated authorization and tracking.</li><li>Notifies required parties within the appropriate timeframe for routine and urgent requests for services.</li><li>Research member history for duplications and consideration of referral limits.</li><li>Provides effective departmental communication with both internal and external sources.</li><li>Forwards referrals to appropriate department staff in terms of eligibility, other coverage, and benefit issues.</li><li>Scans, attaches, reviews and effectively works with electronic images as part of the authorization process. This includes recording the required information from attachments into the authorization fields in NextGen.</li><li>Collaborate with team lead and supervisor to resolve complex referral issues.</li><li>Appropriately forwards all referral requests to the next level of clinical review as applicable and after verifying for completeness and appropriateness.</li><li>Documents and communicates areas of concern to supervisor.</li><li>Adheres to company HIPAA policies and procedures.
