Case Managers work closely with children with disabilities, families, guardians, service providers and natural supports to coordinate support services necessary for children to live in their family home, attend their neighborhood school, work and participate as fully as possible in community life. Education, Training and Experience: Requires a Bachelors Degree in a human services field (social work, psychology, sociology, nursing, counseling, etc.) and one (1) year of professional experience working with children or adults who have developmental disabilities, brain injuries, chronic health conditions, or mental health needs.
