Provides hands on nursing care to residents as needed as a registered professional nurse, consistent with the state’s Nurse Practice Act. Certified Nursing Assistant, Licensed Practical Nurse and Registered Nurse, as needed.

Job Description:Under the supervision of the Assistant Director of Nursing or Director of NursingServices the Unit Manager has authority, responsibility and accountability for the overallmanagement of residents on a designated unit. The Unit Manager is responsible fordirecting, coordinating, and supervising the activities of nursing staff engaged in specificnursing practices and is responsible for transmitting each unit’s staff concerns to theappropriate department supervisor. Demonstrates ability to perform job functions of aCertified Nursing Assistant, Licensed Practical Nurse and Registered Nurse, as needed.Essential Duties1. Perform various duties to provide quality nursing care to residents to maintain orattain the highest practical level of functioning as illustrated by the following:a. Plans effective administration of nursing care in cooperation with other healthrelated disciplines on each unit.b. Responsible for assessment of resident needs, development andimplementation of comprehensive Care Plan. Consults with residents’physician in planning resident care. Periodically reviews and updatescomprehensive Care Plan.c. Evaluates incidents and determines emergency measures to be used. Assessesand directs emergency treatment to residents and staff involved in accidents.If necessary, notifies the physician and the appropriate persons. Completesthe incident report with causes investigated and corrective actions takenand/or suitable recommendations made to avoid recurrence, for review by theFacility Quality Assurance Committee. Updates the Care Plan with newinterventions.d. Supervises planning of daily assignments for personnel on the unit.e. Conforms to established policies and procedures and maintains staffawareness of these in their daily work.f. Schedules and leads staff conferences to promote initiative and solve residentcare oriented problems.g. Works with interdisciplinary team members in developing a discharge plan forindividual residents and assists in achieving discharge planning.h. Assists in orienting and directing new staff.i. Maintains accurate records of resident care and related statistics as required.j. Reviews and interprets work performance against accepted standards.Responsible for completion of performance evaluations of nursing staff. Mayreview and provide input for performance evaluations for nursing staff onalternate shifts. Completes proficiency audits for nurses and assigns andreviews audits for Certified Nursing Assistants.k. Directs requisitions of equipment and supplies and encourages properutilization of same.l. May provide input to the appropriate staff in planning nursing in-serviceprograms and on-the-unit conferences based upon an assessment of staff’slearning needs and/or resident care problems.m. Handles resident or staff grievances and/or refers them to the appropriatedepartment supervisor.n. Is available for emergencies and assistance to nursing staff as needed.o. Communicates and acts as a liaison among residents, family, physicians, andstaff.p. Accompanies physicians on rounds, examinations and treatments, as needed.q. Informs the Charge Nurse and Director of Nursing Services of pertinentinformation concerning resident status, staff actions, employee attitudes andsuggestions.r. Conducts resident rounds to assess and/or supervise nursing care, assesscondition of residents, and promotes a safe, clean, comfortable environment.s. Receives reports from personnel on previous shift and verifies that thecontinuity of care continues by staff on next shift.t. Participates in Nursing Management Meetings, HART Team Meetings, andComprehensive Care Plan Conferences.u. May provide input in formulating nursing policies and procedures.v. Notifies the resident’s physician, family and/or health care agent of anychange in the resident’s condition.2. Establishes and maintains resident care systems per standardized procedure that areconsistent throughout the Facility. These include:a. Unit Management System for group assignments and nursing assistantresponsibilities.b. Restraint Reduction System to verify restraints are at a minimum, and thatrestraints are released at least every two hours, or per Care Plan. Restraintreleases are to occur on the unit at the same time or per Care Plan, and beobserved by a licensed staff member.c. Turning and Positioning System to verify schedules are followed every twohours or per Care Plan.d. Resident Ambulation System to verify residents adhere to their ambulationprogram.e. Scheduling system to verify scheduling for nurses and CNAs is completed in afair and consistent manner.f. Nourishment/Supplements System to verify nourishments/supplements arepassed according to developed schedule.g. Cleanliness System to verify that environment is clean, orderly and organized.Verifies cleaning schedules are in writing and audited daily.h. Infection Control System to verify that appropriate infection control protocolis followed.i. Pharmacy System to verify correct meds are received timely, meds arediscarded timely and properly and costs are appropriate for resident payersources; and med room and med carts are monitored for policy compliance.j. Lab System to verify labs are drawn timely, and in accordance with doctorsorders.k. Reviews Change of Condition/24-Hour Report daily and deletes residentsfrom the report as needed.Standard Requirements1. Supports the Mission, Values and Vision of the Facility.2. Adheres to:a. Confidentiality of all data, including resident, employee and operationsdata.b. HIPAA regulationsc. Quality Assurance requirements and compliance with regulatoryrequirements.d. Compliance with current law and policy to provide a work environmentfree from sexual harassment and illegal and discriminatory behavior.e. Code of Conduct and Compliance Program when performing workfunctions.5. Supports and participates in common teamwork:a. Cooperates and works together with all co-workers; plans and completesjob duties with minimal supervisory direction, including appropriatejudgment.b. Uses tactful, appropriate communications in sensitive and emotionalsituations.c. Follows up as appropriate with supervisor, co-workers or residentsregarding reported complaints, problems and concerns.d. Promotes positive public relations with residents, family members andguests.e. Completes requirements for in-service training, acceptable attendance,uniform and dress codes including personal hygiene, and other workduties as assigned.f. Participates as assigned in the orientation of new staff and demonstrates apositive, helpful, and enthusiastic attitude.6. Immediately reports incidents of alleged resident abuse or neglect or allegedviolations of residents’ rights to Director of Nursing or Executive Director.7. Knowledgeable of age specific developmental factors specific to adult andgeriatric residents (i.e. physical, cognitive and socialization factors) in planningdelivery of care.8. Maintains current practice standards or changes by participating in educationalopportunities provided by the Facility and outside resources.Other DutiesProvides hands on nursing care to residents as needed as a registered professional nurse,consistent with the state’s Nurse Practice Act.Perform tasks which are supportive in nature to the essential functions of the job, butwhich may be altered or redesigned depending upon individual circumstances. Performsother work duties as assigned.QualificationsEducation/Training: Maintains a current, valid license as a registered/licensed professionalnurse in good standing. Must have current CPR certification.A.D.A ((Americans with Disabilities Act)(as amended)) REQUIREMENTS: Must beable to bend, stoop, push, pull, and walk and stand for most of the working day. Requiresmanual dexterity sufficient to provide hands-on care to residents as well as to operatestandard office machines such as fax, telephone, etc. Requires normal range of hearingand vision. Must be free from communicable diseases that pose a "direct threat" to thehealth or safety of others and which cannot be eliminated by a reasonableaccommodation. Must be able to lift a minimum of 50 pounds, and transfer and transportresidents. Must be able to carry and transport supplies. Must be able to pass a post-offer,pre-employment physical health examination as a condition of employment. Must becapable of performing the essential functions of the job, with or without reasonableaccommodation(s).Skill(s): Proficient reading, writing, grammar, and mathematics skills; proficientinterpersonal relations and communicative skills; knowledge of drug interactions, sideeffects, and incompatibilities; decision-making skill; proficient assessment skills; basiccomputer skills; able to operate various medical and office equipment; auditory andvisual skills. Must be able to communicate in English, both verbally and in writing, andpossess sufficient communication skills to perform the tasks required.
