Qualifications:
REQUIRED QUALIFICATIONS<ul><li>Physician Licensed to practice medicine in current state.</li><li>Licensed by the Federal Drug Enforcement Administration and the State of Illinois to prescribe narcotics, if applicable.</li><li>Board certified or board eligible in the appropriate specialty(ies).
