This role involves conducting comprehensive assessments,developing individualized treatment plans, delivering evidence-based interventions, andmaintaining accurate clinical documentation. The therapist will play a vital role in helpingclients achieve their mental health goals and improve their overall well-being. Thetherapist is required to maintain the confidentiality of client information; write andinterpret written clinical information accurately and effectively.Section II: Key Responsibilities Clinical Care & Intervention<ul><li>Deliver direct and telehealth therapy services to clients via phone, video, and in-person.</li><li>Provide crisis intervention and short- and long-term psychotherapy.</li><li>Conduct mental health evaluations and formulate treatment plans.</li><li>Offer referrals and case management services.</li></ul>Documentation & Compliance<ul><li>Maintain up-to-date, accurate clinical records in compliance with HIPAA.</li><li>Ensure all documentation is completed within established deadlines.</li><li>Adhere to mandated reporting protocols.</li></ul>Collaboration & Communication<ul><li>Engage with case management, administrative, and leadership teams to ensure coordinated care.</li><li>Maintain professional, timely communication via email, phone, and instant messaging.</li><li>Participate in team meetings, trainings, and interdisciplinary planning.</li></ul>Work Ethic & Professionalism<ul><li>Work independently with minimal supervision.</li><li>Demonstrate punctuality, reliability, and responsiveness.</li><li>Uphold a high standard of customer service and empathy.</li></ul>Section III: Knowledge & Skills Education<ul><li>A Master's degree from an accredited college or university in psychology, marriage and family therapy, social work, or counseling. </li></ul>Licensure<ul><li>Must hold a current California license as one of the following:</li><li>Associate Marriage and Family Therapist (AMFT)</li><li>Associate Clinical Social Worker (ACSW)</li><li>Associate Professional Clinical Counselor (APCC)</li></ul>Experience<ul><li>Minimum 2 years of experience in treating mental health consumers.</li><li>Case management experience is preferred.</li></ul>LanguageProficiency<ul><li>Fluent in written and spoken English.</li><li>Bilingual in Spanish and/or Arabic is preferred.</li></ul>TechnicalSkills<ul><li>Proficient in Google Suite (Docs, Sheets, Slides) and Gmail.</li><li>Strong written and verbal communication.</li><li>Analytical and multitasking abilities.</li></ul>InterpersonalTraits<ul><li>Compassionate and empathetic approach to patient care.</li><li>Collaborative spirit and professional demeanor.</li></ul>Section IV: Key Interactions Internal Contacts<ul><li>Patients – Direct care and communication.</li><li>Director of Behavioral Health, Chief Clinical Officer, Administrative Staff, Patient Coordinators – Collaboration and care coordination.</li></ul>Core Responsibilities:<ul><li>Establishes positive, trusting rapport with patients.</li><li>Interviews and conducts diagnostic assessments of patients to develop treatment and recovery plans for patients and/or their family members.</li><li>Participates in the intake process by interviewing patients and/or their family members to obtain family, psychosocial, employment, medical, and mental health treatment history.</li><li>Provides crisis intervention, treatment, case management, referrals, and other related mental health care services to patients and their family members.</li><li>Participates in the development of treatment plans designed to address mental, emotional, and family disorders and dysfunction.</li><li>Provides ongoing clinical assessment, treatment planning, and clinical treatment for patients and families.</li><li>Prepares and maintains clinical documentation, in addition to adhering to HIPAA regulations.</li><li>Completes administrative requirements including but not limited to timely completion of documentation by set deadlines.</li><li>Collaborative work with case management team, administrative team, and management team.</li><li>Participates in clinical supervision and maintains supervision log.</li><li>Prompt responsiveness over email, phone, instant messaging, punctuality, and professionalism.</li><li>Willingness to provide mental health services via telehealth to include phone and/or video calls while following mandated reporter protocols.</li><li>Must be able to demonstrate independent work with minimal supervision.</li><li>Collaborating with the Enhanced Care Management (ECM) team on comprehensive case conceptualizations to optimize member outcomes.</li></ul>Work Location: Hybrid;
