Program Summary: Patient Navigators work with various health services providers to assist patients to access appropriate care, establish a medical home, assure pre- and post-visit needs are met and facilitate brief health education encounters related to chronic conditions, medically complicated cases and patients with multiple barriers to clinical care. Sea Mar Community Health Centers, a Federally Qualified Health Center (FQHC) founded in 1978, is a community-based organization committed to providing quality, comprehensive health, human, housing, educational and cultural services to diverse communities, specializing in service to Latinos in Washington State.
