This role is ideal for an RN who is comfortable working independently, making sound clinical decisions in the field, and supporting urgent participant needs with compassion and confidence. Schedule: Saturday: 7 AM – 11 PM Sunday: 7 AM – 11 PM Two 16-hour shifts Responsibilities: <ul><li>Perform in-person participant assessments to determine appropriate levels of care</li><li>Respond to urgent, emergent, and non-urgent participant needs</li><li>Complete regular home visits and field-based nursing visits</li><li>Provide wound care, IV hydration, lab draws, assessments, and other nursing tasks</li><li>Respond promptly to calls from providers, triage, and support staff</li><li>Educate participants, family members, caregivers, and facility staff</li><li>Coordinate with providers, home care, hospitals, pharmacies, DME suppliers, ALFs/SNFs, and other care partners</li><li>Initiate emergency treatment when needed</li><li>Document calls, visits, orders, incidents, falls, medication errors, and quality-related items</li><li>Participate in quality improvement initiatives and team communication</li><li>Adjust to changing workloads and priorities based on participant needs</li></ul> Qualifications: <ul><li>Graduate of an accredited school of nursing</li><li>Current RN license in the State of Michigan</li><li>1–2+ years of home care, geriatric, long-term care, hospice, or field nursing experience preferred</li><li>Experience working with frail elderly or long-term care populations</li><li>Strong clinical assessment and critical thinking skills</li><li>Comfortable working independently in the field</li><li>Strong written and verbal communication skills</li><li>Proficiency with Microsoft Word, Outlook, and EMR documentation</li><li>Current CPR certification or willingness to obtain</li><li>Reliable transportation, valid Michigan driver’s license, and car insurance</li><li>Comfortable driving extensively for home visits</li></ul> Interested in hearing more?
