A Day in the Life of a Hospice Social Worker (Weekend) As a Hospice Social Worker, your weekend shifts are focused on connection and compassion. You’ll meet with patients and families, provide emotional support, and guide them through the hospice journey. This Social Worker role includes collaborating with nurses and the care team, advocating for patient needs, and ensuring comfort during critical weekend moments. What You’ll Do (Hospice Social Worker Responsibilities)<ul><li>Complete psychosocial assessments in hospice care </li><li>Develop care plans and provide counseling as a Social Worker </li><li>Coordinate community resources and referrals </li><li>Support advance care planning (DPOA-HC) </li><li>Maintain timely, accurate documentation </li><li>Collaborate with the interdisciplinary hospice team </li><li>Ensure compliance with Medicare and Illinois standards </li></ul>Schedule (Weekend Role)<ul><li>Saturday & Sunday </li><li>12-hour shifts (8:00am – 8:30pm)</li><li>Consistent weekend schedule for work-life balance </li></ul>What You Bring (Requirements)<ul><li>Master’s Degree in Social Work (MSW) </li><li>Illinois LSW or LCSW preferred </li><li>1+ year experience as a Social Worker (healthcare or hospice preferred) </li><li>Strong communication, empathy, and organizational skills </li><li>Valid driver’s license and reliable transportation </li><li> Local residence with ability to respond within 30 minutes of assigned service area.
