 Promise Point Care is seeking a compassionate and dedicated Case Worker to support and empower youth in a structured, trauma- informed environment. The Case Worker will play a vital role in helping youth develop life skills, achieve personal goals, and successfully transition into independent living. Key Responsibilities <ul><li>Manage a caseload of youth and provide ongoing support services</li><li>Develop and implement individualized service/treatment plans</li><li>Conduct assessments and monitor client progress regularly</li><li>Advocate for youth needs and coordinate with external agencies (schools, counselors, courts, etc.)</li><li>Maintain accurate and up-to-date case documentation and reports</li><li>Provide guidance, mentorship, and crisis intervention when needed </li><li>Facilitate life skills development (education, employment readiness, coping skills)</li></ul>Qualifications <ul><li>Experience in social work, human services, or working with youth</li><li>Experience with at-risk youth preferred</li><li>Strong communication and organizational skills</li><li>Ability to build trust and maintain professional boundaries</li><li>Compassionate, dependable, and team-oriented</li><li>Ability to handle sensitive situations with professionalism</li></ul>Work Environment <ul><li>Youth-centered, supportive environment</li><li>Collaborative team setting</li><li>Opportunity to make a meaningful impact in the lives of youth</li></ul>
