Community Care Physicians, P.C. (commonly called CCP) is the largest, independent multispecialty medical group in the Capital Region of NY. It is a physician-owned, physician-run organization that provides medical care to patients of all ages using state-of-the art technologies. CCP addresses the individual needs and demands of patients seeking primary care, specialty care, urgent care and wellness services. We are not part of a hospital nor are we an insurance company. We are a collection of passionate and skilled doctors, midlevel providers and staff all working together to provide premier medical care for patients in the greater Capital Region area.
