XLHome

XLHome provides in home assessments to qualified members of its contracted UnitedHealthcare Medicare Advantage health plans through HouseCalls visits. During a HouseCall, an XLHome licensed physician or nurse practitioner will complete: a medical history, medication brown bag, review of symptoms, physical exam, screenings, education, diagnoses, flu vaccinations (in some cases) and identification of gaps in care.

The information obtained through the HouseCall is used by the health plan for care management and referral purposes, and is also shared with the member's primary care physician.

Proactive preventative care and care management helps to prevent medical crises before they arise. XLHome is able to provide care in the comfort of the place where the patient resides, avoiding acute inpatient care whenever possible.
