Coordinate management of care and ensure optimum utilization of resources, service delivery, and compliance with external review agencies
Conduct comprehensive assessments, develop care plans, implement plans, and evaluate individual patient needs on an ongoing basis
Integrate case management, utilization review and management, and discharge planning to promote continuity of care and quality patient outcomes
Serve as a patient advocate by addressing holistic patient needs, supporting resource utilization, and facilitating discharge planning across acute care settings
Collaborate with interdisciplinary teams (IDT) to support care coordination and ensure patient preferences are honored
Required Qualifications
Active California RN license required
BLS certification through the American Heart Association required
Minimum 2 years of recent RN Case Management experience in a LTAC or STACH setting
Experience with MCG criteria and discharge planning across acute care settings
Strong interdisciplinary team (IDT) collaboration skills
Preferred Experience / Skills
Prior travel nursing experience
Familiarity with electronic medical record (EMR) systems used in acute or long-term acute care environments
Experience working in a long-term acute care hospital (LTACH) setting
