Title: RN Case Manager
Location: Downey, CA 90242
Duration: 13+ Week Contract (Extension Possible)
Shift: 8x5 | 8:30 AM to 5 PM
Summary: Provides acute care case management services to coordinate patient care, discharge planning, and resource utilization within a hospital setting.
Responsibilities
Perform comprehensive case management, utilization review, and discharge planning for acute care patients.
Coordinate care transitions, authorizations, and services across multidisciplinary teams.
Monitor patient progress and ensure appropriate level of care and compliance with hospital policies.
Requirements:
License: Active California RN license (unrestricted)
Experience: 1+ year current RN Case Management in Acute/Inpatient/Hospital setting
Certifications: BLS (AHA)
