Achieve Behavioral Health has been awarded a NYS Office of Mental Health contract to launch a new 68-capacity Adult Assertive Community Treatment (ACT) team, and were hiring the leader who will build it. This is a senior role for a proven clinician-leader ready to own a program launch end to end; it is not a first leadership position. Achieve is one of New Yorks original CCBHC demonstration sites, a 650-person organization serving more than 5,300 people annually across Rockland and Orange Counties since 1981. This program extends that care to the countys highest-need adults. Why This Role

  • Full backing: A direct reporting line to executive clinical leadership and the full infrastructure of an established organization behind you, from fiscal, HR, and compliance support to vehicles, field technology, and ACT Institute training The Role The Team Leaders job is three-fold: practicing clinician on the team, clinical supervisor to each specialty role, and administrator of day-to-day operations. Time splits roughly 50/50 between direct care and supervisory/administrative work. ACT is an evidence-based, mobile, team-based model serving adults with serious mental illness who havent been well served by office-based care; at least 80% of the teams contacts happen in the community, not the office. What Youll Do
  • Lead a multidisciplinary team of ~9 (nursing, substance use, vocational, family, and peer specialists plus a psychiatric provider), maintaining fidelity to the OMH ACT model and a 10:1 participant-to-staff ratio
  • Facilitate the Daily Morning Meeting; provide individual and field-based supervision grounded in evidence-based practice
  • Build referral relationships with SPOA, hospitals, and community partners; manage a measured admission pace to full census
  • Monitor adherence for participants under court-ordered AOT and coordinate with the county AOT team
  • Oversee documentation quality, CAIRS reporting, chart audits, and the annual fidelity self-assessment; use PSYCKES and program data to drive quality improvement
  • Ensure 24/7 clinical consultation coverage and share in the teams rotating on-call schedule Strong Preference For
  • Prior ACT team experience, ideally as a Team Leader, Assistant Team Leader, or senior clinician
  • Program start-up or new service implementation experience Requirements
  • Masters degree with current NYS licensure (LCSW, LMSW, LMHC, etc.); OR NYS-licensed RN; OR licensed physician
  • 5+ years serving adults with serious mental illness, including 2+ years of supervisory experience
  • Experience on a multidisciplinary community-based team (e.g., ACT, AOT, mobile crisis, intensive care management)
  • Demonstrated competency in evidence-based practices such as motivational interviewing, harm reduction, and trauma-informed care
  • Valid drivers license (agency vehicles provided); pre-employment clearances required, including fingerprinting and NYS Justice Center Staff Exclusion List check Compensation & How to Apply Salary of $95,000-$105,000 commensurate with experience, plus comprehensive benefits (medical, dental, life, retirement, PTO), full ACT Institute training support.
