The Admission Nurse is a clinical professional responsible for initiating and resuming high‑quality home health services for patients referred to the agency. This role conducts comprehensive home assessments to determine skilled care needs, establishes an individualized plan of care in collaboration with the interdisciplinary team, and ensures timely, accurate documentation that meets regulatory, payer, and agency requirements.
The Admission Nurse provides patient and caregiver education, evaluates home safety, and confirms eligibility for services. Through strong clinical judgment, excellent communication skills, and a patient‑centered approach, the Admission Nurse helps ensure a smooth transition into home care and sets the foundation for positive patient outcomes.
