Patient Assessment: Performs comprehensive evaluations, including physical assessments and health histories, to determine patient needs.Medication Reconciliation: Ensure accurate medication list are maintained, identifying any discrepancies.Care Plan Development: Accurate documentation of patient information and care plans is essential for continuity of care.Patient Education: Explain facility policies and procedures to residence and families, addressing any concerns.
