Advanced Practice Provider Supervisor

Job Description: Community LIFE is a Program of All-Inclusive Care for the Elderly (PACE) dedicated to helping older adults live safely and independently in their homes and communities. We are seeking an experienced Nurse Practitioner or Physician Assistant to serve as our Advanced Practice Provider Supervisor. This NEW leadership role combines clinical practice with team development, supporting APPs across our service areas while helping older adults remain independent and healthy in their communities. This position will be based out of our Forest Hills, PA (15221) location but will cover all of our service areas, so will require local travel with mileage reimbursed. For details about the areas we cover, please visit www.commlife.org/locations What You'll Do Lead, mentor, and support the Advanced Practice Provider (APP) team. Promote a positive team culture and clinical excellence. Collaborate with physicians and interdisciplinary team members to ensure high-quality participant care. Support recruitment, onboarding, and professional development of APP staff. Provide direct participant care and coverage as needed. Conduct assessments, manage chronic and acute conditions, and coordinate specialty referrals. Participate in care planning, quality improvement initiatives, and regulatory compliance activities. Work Monday-Friday, 7:30am-4:00pm and provide limited on-call coverage as part of a rotation of providers What We're Looking For Proven clinical and leadership skills Strong communication and team-building abilities Commitment to participant-centered care Ability to work effectively within an interdisciplinary healthcare team Passion for serving older adults Required Education Nurse Practitioner: Master's, Post-Master's, or Doctoral NP program Physician Assistant: Graduate of an accredited PA program Required Experience 5+ years as a Nurse Practitioner or Physician Assistant 3+ years of leadership or supervisory experience Experience with geriatrics, aging services, or PACE preferred Required Licensure Current Pennsylvania NP or PA license Prescriptive Authority and DEA registration Valid Pennsylvania driver's license All licenses must be active and unrestricted Why Community LIFE?
