My AmeriCorps - Home Page AmeriCorps VISTA - Program Sustainability & Community Access
.The mission of the Pamela Parker Community Outreach Health Clinic (COHC) is to provide quality, non-emergency medical, dental, and pharmaceutical care; connect patients to appropriate community services; and promote health and wellness through education and counseling. COHC serves uninsured and underinsured individuals living within the Froedtert Health Menomonee Falls Hospital service area through a combination of paid staff and volunteer healthcare professionals. As the clinic continues to grow, COHC relies on effective systems, clear policies, and a strong volunteer infrastructure to meet increasing community needs. To ensure long-term sustainability and consistent access to care, the clinic requires strengthened policies, improved data-informed processes, and enhanced systems to support volunteers and program coordination. The AmeriCorps VISTA member will focus on capacity-building activities that strengthen clinic operations and organizational sustainability. Key areas of focus include reviewing and updating clinic and volunteer policies and procedures; supporting the development of training and reference materials; creating systems to collect and analyze patient and service data to identify gaps and priority populations; strengthening communication and tracking processes for health monitoring initiatives; and supporting systems for onboarding, training, and coordination of student and professional volunteers. The VISTA member will also assist with documenting and developing sustainable chronic disease education and care coordination resources. By strengthening internal systems and infrastructure, the VISTA project will expand COHC's organizational capacity to meet the long-term healthcare needs of underserved populations beyond the VISTA service term.
