Assessment Care Manager SHC - Internal posting

Basic Function:           To assist older adults to obtain services that will secure an independent lifestyle.  To identify gaps in services and advocate for services that will maximize independent functioning. To educate and promote awareness of public benefits and community services.

Responsibilities:

1. Conduct an initial assessment of physical, social, environmental and emotional status to determine needs and eligibility requirements within AGE time frame guidelines.
2. Develop service plan and supportive networks through client, formal and informal supports, and consultation with supervisor.             
3. Assist client to obtain and utilize other community services and public benefits such as: VNA, housing, fuel assistance, council on aging, Medicaid, SNAP, etc.   
4. Participate and recommend consumers for risk and ICR meetings as needed.                                                                                            
5. Provide support and information to consumers and those directly involved in consumer’s care.  Encourage self-advocacy.                  
6. Set up and prepare the initial client file. The electronic files should include copies of all correspondence with client.
7. Keep journal notes up to date, including summary of telephone contacts, service changes and calls concerning clients.       
8. Verify client file information for the annual hand count for EOEA.                                                                                    
9. Update knowledge pertaining to gerontology and services for elders through courses, and in services.                       
10. Participate in staff meetings to discuss home care and other related issues.                                                                                     
11. Meet with supervisor on a regular basis.       
12. Perform miscellaneous duties and assignments as directed by supervisor.
