AZ-MSU - Discharge Planner

Job Description:

DESCRIPTION: Performs case management and coordinates the discharge planning care process of age-appropriate in-patients with chronic diseases and special health care needs within Multi-Service Unit, Intensive Care Unit, OB Units and Adolescent Care Unit by maintaining a tracking system for referred patients for admission with other healthcare facilities by performing chart reviews for requested services for return to facility or discharge home.SCHEDULE: 8-Hour Shift, Monday through Friday, 8:00 a.m. to 5:00 p.m. (Travelers are required to attend staff meetings, when scheduled)REQUIREMENTS: Two (2) Years or more experience in discharge planning and Med-Surg experience. Must have RN experience in a clinical, medical/surgical, acute and/or critical care setting.LICENSE/CERTIFICATIONS: Valid and unrestricted Registered Nurse license in any U.S. State or Territory. Valid American Heart Association (AHA) certification in Basic Life Support (BLS) and must maintain certification.HOUSING: Housing is limited, but information can be found under the "Facility Group Documents" section.ORIENTATIONS: All Travelers are required to attend New Employee Orientation and is scheduled every other Monday. Travelers are required to be cleared 7-day prior to their start date, and if they are not cleared their start date will be pushed to the next schedule NEO Monday.
