Bilingual Care Coordinator

Now Hiring: Bilingual Care Coordinator - Children's Long-Term Support (CLTS) Program

Lutheran Social Services of Wisconsin & Upper Michigan (LSS) is seeking a Bilingual Care Coordinator to join our growing Milwaukee County CLTS team. This role requires fluency in both Spanish and English.

The Childrens Long-Term Support (CLTS) Waiver Program is a federally funded Medicaid program that supports children with developmental, physical, and severe emotional disabilities. As a Care Coordinator, you will partner with families to assess needs, coordinate services, develop individualized care plans, and connect children with resources that help them remain safely in their homes and communities.

Schedule: Full-time, Monday-Friday, first shift

Work Environment: Hybrid schedule with up to 2 remote workdays per week

What Youll Do

• Complete assessments to identify client and family needs.
• Develop and implement person-centered service plans in collaboration with families, support teams, and providers.
• Coordinate and monitor services to ensure goals and outcomes are met.
• Facilitate family and team meetings.
• Maintain accurate documentation and client records.
• Connect families with community resources and supports.
• Provide services utilizing trauma-informed and family-centered practices.
• Collaborate with service providers, community partners, and internal teams.

Qualifications

• Bachelors degree in a human services-related field such as Social Work, Psychology, Counseling, Sociology, Special Education, Criminal Justice, or a related discipline.
• At least one year of experience working with children or individuals with developmental, physical, or behavioral health needs.
• Fluent in Spanish and English (required).
• Valid drivers license, reliable transportation, and ability to travel throughout Milwaukee County.
• Strong communication, documentation, organizational, and computer skills.
