Strategic Function: Provide comprehensive case management services that will support families to foster their child's development and assist families to attain their personal goals, maintain appropriate files and complete necessary reports. Conducts home visits using Parents as Teachers curriculum. Conduct home visits to young children and their families utilizing a research based (PAT) curriculum bi-weekly or monthly / 60-minutes per visit. Complete documentation of home visits, group connection experiences, and other relevant activities.Reports to Parents as Teachers Coordinator. Operating Principles: A. Develop and maintain positive, open and productive team partnerships with families, community partners and staff while maintaining professional boundaries. 2. Adhere to the Social Work Code of Ethics. 3. Relying upon the family’s strengths, work with the family to assess needs and assure that all interventions promote family empowerment, self-sufficiency and self-advocacy. 4. In partnership with families, develop a realistic short term and long-term goals and the appropriate strategies to achieve these goals. 5. Promote family literacy to children ages 0-5 in the family home. 6. Make referrals to needed resources and assist families in accessing resources as necessary and appropriate. 7. While following all Transportation Procedures; assist with transporting families to appointments as needed. 8. Advocate for families and the Home Visiting Program in the community. 9. Ensure families comply with the Home Visiting Program Participation and Consent form including attendance home visits and group connections. 10. Obtain and maintain Parent’s as Teachers (PAT) certification at agency’s expense and provide parents with the opportunity to receive bi-weekly or monthly home visits to implement PAT curriculum as required. 11. Continually update Community Resource information. 12. Meet monthly with the Home Visiting Program Coordinator to assign priorities to families on your case load (more frequently as needed). 13. Maintain accurate documentation on families by funder and Home Visiting model standards. 14. Inform Parents as Teachers Coordinator of family emergencies in a timely manner. 15. Report all suspected child abuse to the Child Abuse Hotline at 1-800-252-5400 and to the Parents as Teachers Coordinator. 16. Assist in gathering recruitment data throughout the year. 17. Be active in ongoing self and program assessment in order to participate in continuous improvement of your own skills and program enhancement. 18. Promote parental involvement in curriculum planning and overall program activities. 19. Conduct developmental screening and ongoing assessment of young children’s development and family well being. 20. Allowing flexibility with work schedule and location as needed based on program needs, family availability and other job requirements. 21. Facilitate monthly group connection experiences and participate in ongoing outreach and recruitment events in the community. 22. Maintain maximum caseload in addition to all other job expectations while in a fully remote position. General Indicators:
