Job Description

Must have at least a Bachelors in Social Work, Mental Health Counseling, Psychology, Sociology, or related field. The Qualified Mental Health Professional/Case Manager will be responsible for providing Targeted Case Management and Rehabilitative Services to youth with serious emotional disturbances and their families.


Job Description


The mission at Diamond Experience Co. is to provide high quality, gentle and compassionate outpatient services to children, adolescents and adults on their journey to mental wellness. The Qualified Mental Health Professional (QMHP) will provide goal directed interventions to stabilize children and youth in transition, and follow a strengths-based model that will help develop and build skills that will help the child and family function autonomously and successfully.


Requirements


  • CANS/ANSA Certification
  • Bilingual- English and Spanish required
  • Ability to pass drug test and background check
  • At least two (2) years prior experience working in the behavioral health field or closely related field
  • Must have reliable transportation, proof of valid driver’s license, auto insurance and clean driving record
  • Must have access to computer and internet
  • Ability to work efficiently, effectively, and independently with minimal daily oversight
  • Competent in using Microsoft Word, Excel, Power Point and EHR software
  • Ability to work from a strengths-based, trauma–informed, and culturally competent perspective
Responsibilities


  • Must acknowledge and adhere to Medicaid/Insurance guidelines
  • Work closely with the clinical team in developing a service/treatment plan that is focused on the child and family needs and recognizing their strengths
  • Responsible for providing skill-building training in the home or out in the community using evidence-based practices and curriculums
  • Provides support and crisis/emergency services for the child/family through face-to-face contact, phone or virtual
  • Maintain all client documentation in the EHR and write progress notes adhering to requirements set by the Texas Administrative Code
  • Enter data in a clear, accurate, and timely manner. Completion of daily session notes within 24 hours after session
  • Documents non-billable contacts with the clients to show history of follow-up and/or non-compliance
  • Updates necessary client demographics information in EHR software by the 20th of each month
  • Promptly submits documentation needed for the tracking of output/outcome measures. May be asked to assist in record review or qualitative review processes.
  • Utilize effective listening skills to establish rapport with clients, family members, and other individuals close to the client; provide emotional support and encouragement; and engage family members in the service process
  • Responsible for keeping the family and team members informed on progress toward achieving Service Plan goals
  • Assist clients with daily living tasks such as behavior management, counseling services to the client, budget management, independent transportation assistance, time management skills, and other tasks to promote independent living
  • Collaborate with court/probation, tribal/community agencies, families, and other natural support systems to ensure client’s needs are met. Develops a strong working relationship with community partners. Assist clients in practicing effective communication, problem-solving, and anger management techniques in their daily lives
  • Participate in staff meetings, monthly training, company programs, professional development training as required
