Pay Range: $29 /hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
Key Responsibilities:- Connect members to community and care resources in a cost-effective manner.
- Conduct telephonic and in-person outreach to schedule screenings and appointments.
- Perform screenings using a retinal eye camera and a bone mineral density (BMD) scanner.
- Document and maintain community resources to ensure compliance with health plan requirements.
- Support the clinical team with non-clinical general health assessments and member inquiries.
- Engage in community education and outreach initiatives.
Key Requirements and Technology Experience:- Familiarity with Social Determinants of Health (SDOH) barriers.
- Experience in community outreach and education.
- 1–2 years of relevant experience, preferably as a Medical Assistant or in patient care.
- Hands-on experience operating a retinal eye camera and a bone mineral density (BMD) scanner.
- Valid driver’s license and the ability to travel within Wayne County, Michigan.
- High School Diploma or GED.
