General Description CADCTo provide individual and group counseling for the addicted population who are on medically-assisted treatment. Intake, assessment and case management services, ongoing assessment for program eligibility, monitoring all aspects of care and client services, and psycho-educational classes may be included at discretion of Clinical Director.Job Requirements (includes but may not be limited to)Must have an active/current CADC licenseKnowledgeable in the drug and alcohol fieldHave a positive attitude with clients servedMaintain professionalism and have a constructive approach to problem-solvingProviding alcohol and substance abuse counseling in individual and group formatsFacilitate required Cognitive Behavioral Intensive Outpatient groups and classesDevelop a comprehensive treatment plan that meets the needs of the clientNJSAMS intake assessmentProvide the client with information on community services and agency services which will address their needs and make the appropriate referrals on behalf of the clientUtilization of case management skillsParticipates in case conference with clients and present cases at the Multidisciplinary Team meetingsDischarge planningMaintain client chart according to licensure standardsEnsure all services are supported by care plan, and case note documentationMaintain a caseload of at least 50 clientsEnsure that all court stipulations are met by each client assigned to the caseloadEnsure that all the duties of this position are performed in accordance with licensure standardsQualifications/Skill RequirementsCADC licenseA valid driver's licenseWorking knowledge of NJSAMS preferredGood organizational skills
