My AmeriCorps - Home Page Care Coordination and Outreach Services at MSM
Madison Street Medicine (MSM) provides street-based medical outreach and care coordination for individuals experiencing homelessness and housing instability. Through volunteer-driven care and strong community partnerships, MSM works to improve access to healthcare and connect individuals to ongoing services that support long-term stability. MSM continues to expand its reach and is seeking to strengthen core capacity-building systems to support sustainability and growth. This includes formalizing policies and procedures, improving communication systems, strengthening volunteer and community engagement, and enhancing resource development efforts. Member Duties : Provide direct services to Madison Street Medicine clients through care coordination, outreach, and office walk-in support. Reliable access to a vehicle and comfort transporting clients is required. Member will follow up on client needs after shelter clinics, assist clients in establishing primary care and obtaining health insurance, coordinate and provide transportation to medical appointments, support volunteer transportation efforts, and obtain medications and medical supplies for clients. Member will participate in street outreach activities, helping connect clients to healthcare services and insurance while distributing basic necessities such as food, water, and hygiene supplies. Member will also provide office walk-in coverage by assisting clients with clothing and other basic needs and supporting staff during office hours.
