Job DescriptionThis position is in support of the County of Sarasota Florida to provide care for those with special needs who are displaced in the event of a large-scale disaster. The position requires the individual to be able to respond to a disaster within 24 hours of notice and to establish care within 72 hours of notification. This is a post‑disaster mission and will require the individual to be onsite until the conclusion of the operation.The Care and Discharge Planner is responsible for coordinating and managing the discharge process for our residents, ensuring a seamless transition from the shelter to their next level of care or placement.Key ResponsibilitiesThe Discharge Planner will work closely with healthcare providers, residents, and families to develop and implement comprehensive discharge plans that meet individual needs and comply with regulatory standards.Arrange and coordinate post‑discharge services, including home health care, outpatient therapy, and community resources.Communicate discharge plans effectively to residents, families, and staff, ensuring clarity and understanding of next steps.Maintain accurate and up‑to‑date documentation of discharge planning activities in compliance with regulatory standards and policies.Ensure discharge plans meet all applicable state and federal regulations, including Medicare and Medicaid requirements.Provide education and support to residents and families regarding post‑discharge care options and resources.Listen to patients regarding any health concerns they might have and report details to a Registered Nurse or Medical Provider (MD, PA, NP).Complete required organizational compliance education, including client‑specified assignments for Joint Commission Healthcare Staffing Services certification or other regulatory bodies.QualificationsExperience RequiredMinimum of 2 years of experience in discharge planning, case management, or a related role in a healthcare setting, preferably with special needs populations.Strong oral and written communication skills.Excellent interpersonal skills.Critical‑thinking skills.Cultural competency.Integrity and honesty.Experience with special needs populations (preferred, not required).Education RequiredBachelor's degree in social work or a related human services field such as sociology, special education, rehabilitation counseling, psychology, etc.Graduate from an accredited nursing program for Licensed Vocational/Practical Nurse located in the State, the District of Columbia, or a U.S. territory accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE).Required Licenses / CertificationsCurrent, full, and unrestricted license as LPN/LVN in the state of Florida.Basic Life Support certification.Current and valid driver's license.Required Knowledge, Skills and AbilitiesUnderstanding of “Special Needs” populations (preferred).Strong knowledge of Medicare, Medicaid, and other insurance regulations related to discharge planning.Ability to navigate an electronic work environment, including electronic health records, web‑based training, and communications.Knowledge of, and moderate proficiency in, common Microsoft Office programs, specifically Word, Excel, Outlook, and SharePoint.Knowledge of HIPAA/Privacy Act regulations regarding the confidentiality of patient medical records and information as well as Personally Identifiable Information (PII).Demonstrate strong organization and time‑management skills.Cultural competency and interpersonal skills.Duty Hours / ON-Call RequirementsThis is an upon‑notification position.Shifts will be 12 hours a day until complete.Physical DemandsPhysically capable of performing duties and responsibilities as listed above.Ability to work in an austere environment; exposure to heat and extreme weather conditions.Frequent use of common office technology such as computer, telephone, etc.Ability to lift, carry, and move up to 30 lbs.Ability to drive a vehicle on behalf of the company.Travel / Rotation RequirementsTravel to Sarasota upon notification.BenefitsFull‑time positions are eligible for a comprehensive and competitive benefits package including medical, dental, vision, and basic life insurance.
