    Job Description

    Position Summary

    This position is funded by a grant for a duration of time as defined by the grantor.

    Works under the general supervision of the CHANT program leadership team and provides care coordination services to improve health outcomes for vulnerable populations. This includes pregnant women and children under the age of 21 who are either eligible for or enrolled in TennCare, as well as individuals with special healthcare needs.

    Pay Grade: 48

    Salary Commensurate with Experience and Education

    Minimum Qualifications

    • Two (2) years of case management or care coordination experience; AND
    • Bachelor's degree from an accredited college or university in public health, business administration, business management, social work, sociology, psychology, public health, health administration, or a closely related field; OR
    • An equivalent combination of related education and/or experience.
    • PROOF OF EDUCATION, TRAINING, AND/OR EXPERIENCE IS REQUIRED.

    Duties and Responsibilities

    • Provides care coordination services in accordance with CHANT/CSS guidelines, grant requirements, and mandates for children eligible for medical services.
    • Develops and implements individualized plans of care, assists families in navigating health and social services, and supports goal setting within established care pathways.
    • Connects individuals and families to appropriate health and community resources and provides follow-up to ensure continuity of care.
    • Educates and empowers underserved populations, including pregnant women and families, to improve maternal and child health.
    • Conducts screenings, assessments, and required home visits to identify, validate, and address client needs.
    • Coordinates medical follow-up with families and providers via telephone, mail, home visits and collaboration with community partners.
    • Accurately enters and maintains client data and records in State and Health Department databases and prepares required reports in compliance with state and county guidelines.
    • Builds and maintains partnerships with community organizations and participates in targeted outreach, training, and public health emergency response activities.
    • Performs other related duties as required or directed.

    • Ability to exercise sound judgment and make effective decisions while providing care coordination services in accordance with program guidelines, mandates, and grant requirements.
    • Knowledge of care coordination and case management best practices, including program goals, policies, and procedures, to support individualized plans of care and service navigation.
    • Ability to assess client needs and apply analytical and problem-solving skills to guide care planning, follow-up, and program quality improvement.
    • Strong oral, written, and interpersonal communication skills for educating clients, collaborating with partners, and supporting outreach activities.
    • Knowledge of community resources and health and social service agencies to connect individuals and families with appropriate services and supports.
    • Ability to pay attention to detail and technical proficiency, including working knowledge of medical terminology and proficiency with Microsoft Office and database systems for accurate documentation and reporting.

    Special Requirements

    • Must possess a valid Tennessee driver''s license upon date of employment.
    • Must have daily access to an operable automobile and meet Shelby County requirements for automobile insurance upon date of employment.
    • Must be willing to carry and use a cellular phone for work-related communication.
    • Must be able to travel regularly to off-site locations, including homes, community agencies, schools, medical offices, community events, grantor meetings, and secured correctional facilities.
    • Must attend required field staff meetings, grantor training sessions, and conferences, including those requiring in-state and out-of-state travel.
    • Must obtain Community Health Worker certification within one (1) year of employment.
    • Must maintain all required licenses, certifications, and training credentials throughout the duration of employment.

    Physical Requirements

    • Involves physical activities like standing, walking, climbing, reaching, and repetitive motions.
    • Occasionally, the individual must apply up to 25 pounds of force, and frequently up to 10 pounds.

    Shelby County Resident Disclaimer

    All employees hired after September 1, 1986, must be residents of Shelby County and shall continue to reside in the County as a condition of their employment. The residency requirement shall not apply to certain public safety/civil service employees. This means exemption from this policy is in effect for the following departments and positions: (1) Sheriff Deputy Patrol Officers, Deputy Jailers and Dispatchers, employed by the Sheriff Department, (2) Correction Officers employed by the Division of Corrections, (3) Firefighters, Paramedics and Dispatchers employed by the Shelby County Fire Department. The residency exemption for public safety/civil service employees does not include appointed positions with the Sheriff's Office, Division of Corrections, or the Fire Department.
