    Position Overview

    We are seeking a certified Texas Emergency Medical Technician-Basic (EMT-B) to serve as a Care Coordinator supporting our Start of Care Operations Team. This unique community-based role supports home health operations by conducting drive-by visits in order to locate patients who are unreachable by phone. The ideal candidate is a compassionate, self-directed professional comfortable working independently in the field.

    Key Responsibilities

    Conduct scheduled and unscheduled in-person drive-bys on potential home health patients who are non-responsive to phone contact attempts by start of care (SOC) operations team.

    Facilitate and support start-of-care (SOC) processes by confirming patient presence, availability, and readiness for initial skilled nursing or therapy evaluation

    Serve as a liaison between the patient and the home health care team to reduce gaps in care and prevent unnecessary hospitalizations

    Assist in verifying patient demographic and insurance information at point of contact

    Communicate findings in real time to the SOC Operations Team, Clinical Manager, and Sales Team member through appropriate communication channels

    Perform basic assessments within EMT-B scope of practice to identify any immediate safety concerns or urgent medical needs

    Coordinate with Clinical Manager when a patient is found in acute distress or requires potential emergency intervention

    Maintain confidentiality and comply with all HIPAA regulations

    Required:

    Current, active Texas EMT-Basic certification

    Valid Texas Texas Driver's License with Valid Auto Insurance

    Reliable personal or agency-provided vehicle (mileage reimbursement available)

    Strong written and verbal communication skills

    Ability to work independently with minimal supervision

    Preferred:

    Prior experience in home health, community paramedicine, or EMS

    Bilingual (English/Spanish) strongly preferred given patient population

    CPR/BLS certification (American Heart Association)

    Work Environment & Schedule

    Field-based role covering assigned geographic territory

    Full-time or part-time options available; may include early morning, evening, or weekend shifts to reach patients at home
