    Position Details:

    Job Summary:

    In collaboration with the interdisciplinary team, Care Manager Associates are responsible for supporting the Care Management team in effective transition planning and length of stay management.

    The Care Management Associate is responsible for coordination of information and communication and other clerical functions for the Care Management team.

    The focus of their efforts is to perform duties and responsibilities in a manner consistent with our mission, values, and Mercy service standards.

    The position brings a unique opportunity for an independent, motivated individual who has interest in coordinating and collaborating with healthcare team members to ensure patients receive the right information at the right time.

    Job Description:

    Provides the clerical component of the care management patient care model to insure that department services support coordination of patient throughout.
    1.1 Communicates patient/family concern or issues with Care Managers and Social Workers to ensure continuing care needs are addressed
    1.2 Provides assistance and support for Care Management team
    1.3 Assist Care Managers and Social Workers by delivering and retrieving prescriptions to Mercy Tower Retail Pharmacy
    1.4 Obtains quotes from transport companies regarding cost of transportation from Mercy to post acute destination
    1.5 Delivery of patient choice letters to patient/family
    1.6 Delivery of facility lists to patient/family
    Actively supports care management plans to insure that department services meet Mercy facility needs, customer service needs and care management metrics.
    2.1 Prints and distributes IMM letters to patients and documents appropriately in Epic EMR
    2.2 Checks for consult orders in the Epic EMR and informs Care Manager/Social Worker of the order in a timely manner
    2.3 Uses information and creative thinking skills to identify and resolve problems by actively participating in the development of solutions and implantation of action plans to resolve problems, increase effectiveness and efficiency, maximize resources, remove waste, and/or decrease cost
    2.4 Work on performance improvement plans to increase co-worker and patient satisfaction

    Exhibits behavior and adopts practices to ensure optimal risk management and financial stability.
    3.1 Identifies potential risk management or clinical quality/safety issues and resolves them through intervention or collaboration with other members of the healthcare team as appropriate or escalates the issue to the appropriate individuals.
    3.2 Observes standard precautions consistently.
    3.3 Wears proper personal protective equipment (PPE) when appropriate.
    3.4 Maintains knowledge of and adheres to current Mercy facility and Care Management department policy and procedures.
    3.5 Actively participates to ensure overall compliance with accreditation agency standards of care, State and Federal regulations.

    Communicates effectively.
    4.1 Resolves situations as appropriate within scope of job.
    4.2 Handles complaints/concerns in a prompt courteous manner; assesses and directs to appropriate person to deal with concerns as needed; makes attempt to solve issues to solve issues with involved parties.
    4.3 Keeps leadership informed of problem situations.
    4.4 Actively participates in co-worker meetings for information exchange, discussion of performance indicator results and action planning.
    4.5 Actively participates in co-worker surveys.
    4.6 Demonstrates support for administrative decisions.
    4.7 Develops professional relationships with hospital and community partners.
    4.8 Represents Mercy Care Management in a professional manner at all times.

    Demonstrates ethical behavior and provides confidentiality in all interactions.
    5.1 Identifies and reports ethical situations to leadership for resolution.
    5.2 Respects patients/guests/co-workers personal property.
    5.3 Maintains confidentiality regarding personnel and strategic plans.
    5.4 Maintains confidential Care Management interaction with vendors and patient/family at all times.
    5.5 Adheres to Mercy HIPPA policy and procedures.

    Qualifications/Education:
    - High School Diploma, minimum required
    Licensure:
    - None
    Experience:
    - 1 year Healthcare experience, may include Healthcare student experience preferred - Recent hospital experience, Preferred.
    Certification/Registration:
    - BLS (CPR) at hire date, Minimum required, or within 90 days of hire
    Other Skills and Knowledge:
    - Excellent interpersonal and organization skills, Minimum required
    - Ability to work with and preserve confidential information, Minimum required
    - Ability to deal effectively with diverse groups of people, Minimum required
    - Ability to work independently and in a self directed manner, Minimum required
    - Strong verbal and written communication, and facilitation skills, Minimum required
    - Strong time management skills, prioritization skills, and critical thinking skills, Minimum required
    - Knowledge of Epic, preferred

    About Company

    Our Mercy health system was founded by the Sisters of Mercy in 1986. But our heritage goes back more than 185 years. It began with an Irish woman named Catherine McAuley, who wanted to help the poor women and children of Dublin. Though Catherine had a modest upbringing, she received an unexpected inheritance that allowed her to fulfill her dreams. In 1827, she opened the first House of Mercy in Dublin, intending to teach skills to poor women and educate children. Many volunteers came to help. A few years later, Catherine founded the Sisters of Mercy, the first religious order not bound to the rules of the cloister, whose Sisters were free to walk among the poor and visit them in their homes. By the time Catherine died in 1841, there were convents in Ireland and England, and in 1843, the Sisters of Mercy came to the United States.
