Manage daily operations and staff in Care Management Department, including care coordination, case management, discharge planning, utilization management, social work services, and pre-admission case management.
Act as liaison between patients, families, department staff, physicians, healthcare team, community agencies, managed care representatives, and skilled nursing facilities.
Establish, implement, and monitor best practices and clinical standards to ensure quality patient care and organizational financial integrity.
Required Qualifications:
Master's Degree in Social Work from an accredited school.
Washington Clinical Independent Social Worker License.
6 years experience in Case Management, including acute care setting.
2 years Managerial/Director level experience, preferably in Case Management in a large hospital.
