Care Manager

Pro Care by Abound Health is seeking a Care Manager to join our team in Southfield, MI.

You’ll lead client care coordination, support families and staff, and ensure high-quality service delivery for individuals receiving Michigan Waiver services. This is a hands-on, impactful role where you’ll drive outcomes and make a real difference in people’s lives.

Key Responsibilities:

• Manage assigned client cases, ensuring services are delivered accurately and on time
• Review and maintain all client and staff documentation to meet regulatory standards
• Conduct client and staff meetings to ensure compliance with care plans and policies
• Develop and track individualized goals, interventions, and outcomes
• Coordinate staffing schedules and ensure proper coverage for all services
• Serve as the main point of contact for clients, families, and community partners
• Resolve concerns, document incidents, and ensure quality care delivery across all cases

Qualifications:

• Experience in care coordination, case management, or related healthcare/social services field
• Strong organizational skills and ability to manage multiple priorities in a fast-paced environment
• Excellent communication and relationship-building skills
• Detail-oriented with strong critical thinking and problem-solving abilities
• Ability to work collaboratively with clients, families, and interdisciplinary teams
• Proficiency in Microsoft Office and documentation systems

Education:

• Master’s degree + 1 year experience working with individuals with disabilities, OR
• Bachelor’s degree + 2 years of relevant experience, OR
• Associate’s degree/60 credits + 4 years of experience, OR
• High School Diploma/GED + 5+ years of relevant experience

Compensation & Benefits:

Base pay is determined by factors such as location, experience, skills, and education. In addition to base pay, we offer a comprehensive benefits package including medical, dental, and wellness benefits starting on your 90th day of employment.
