About the Position

We are seeking a PRN Occupational Therapist (OT) to join our skilled nursing facility team. This is a great opportunity for an OT looking for flexible scheduling, supplemental income, and a rewarding clinical environment.

The OT will provide skilled occupational therapy services to residents with a variety of medical, orthopedic, neurological, and functional needs.

Responsibilities

• Complete occupational therapy evaluations and develop individualized treatment plans.

• Provide skilled OT treatment to improve ADLs, functional mobility, transfers, strength, balance, coordination, cognition, and independence.

• Monitor resident progress and adjust treatment plans as needed.

• Complete timely and accurate clinical documentation.

• Participate in interdisciplinary care planning and collaborate with nursing, PT, SLP, physicians, and other healthcare professionals.

• Educate residents, families, and caregivers on safety, adaptive techniques, and equipment.

• Recommend and train residents in appropriate DME and adaptive equipment.

• Provide compassionate, resident-centered care while maintaining compliance with facility and regulatory requirements.

Qualifications

• Current Occupational Therapist (OT) license in KY

• Graduate of an accredited Occupational Therapy program.

• SNF, geriatric, inpatient rehab, or post-acute experience preferred.

• Strong clinical, communication, and documentation skills.

• Ability to work independently and as part of an interdisciplinary team.

• CPR/BLS certification, if required.
