About the Role:
Transitional Residence for Youth is looking for a compassionate and driven Case Coordinator to join our team in Topeka, KS. In this meaningful role, you'll play a vital part in guiding young people through critical transitions, connecting them with the resources and support they need to thrive. Responsibilities:

• Develop, implement, and monitor individualized case plans for youth residents
• Coordinate services and referrals to community resources, including mental health, education, and employment support
• Conduct regular check-ins and assessments to track client progress and adjust care plans as needed
• Maintain accurate and up-to-date case documentation and client records in compliance with program standards
• Collaborate with internal staff, external agencies, and family members to ensure holistic support for each youth
• Facilitate life skills workshops and group sessions to promote independence and self-sufficiency
• Advocate for youth needs within the organization and with outside service providers

Requirements:

• Bachelor's degree in Social Work, Psychology, Human Services, or a related field preferred
• Prior experience in case management, youth services, or social services strongly preferred
• Knowledge of trauma-informed care and youth development principles
• Strong organizational skills with the ability to manage multiple cases simultaneously
• Excellent written and verbal communication skills
• Ability to work collaboratively with a multidisciplinary team and community partners
• Valid driver's license and reliable transportation required
• Empathetic, patient, and dedicated to empowering at-risk youth
