We have an exciting opportunity to join our team as a Case Management Coordinator - Palliative Care - Full Time Days - Palliative Care - Full-Time - Monda.
In this role, the successful candidate The NYULH-Suffolk Palliative Care Team will provide patients with the highest quality medical care available, focusing on comfort and compassion. The team is committed to providing patient and family centered care that optimizes the quality of life by anticipating, preventing, and alleviating pain and suffering. A multidisciplinary and collaborative team approach model will be used, applying the professional core competencies of palliative care to include but not limited to; pain and symptom management, emotional, psychological, spiritual, family support, communication, and teaching.
The Palliative Care CM Coordinator will build a relationship with the patient, family and their support system, and will assist the patient and family in areas of need they have identified together.
This position will be responsible for departmental utilization review and CQI.

Minimum Qualifications:
To qualify you must have a MSW
Current New York State license to practice, LCSW preferred.
Prior experience in palliative/hospice care/behavior health preferred
Knowledge of federal, state and local regulations
Experience in Quality Management, Utilization Review, Case Management
Possesses current knowledge of CQI processes.
Excellent verbal and written communication skills
Must possess a New York State Drivers license..
