Location: Brooklyn, NY
Pay:$25–$26/hour
Schedule: Full-Time
Assignment:Long-Term Temporary Role
Bilingual Russian:Preferred
Education:Bachelor’s Degree in Social Work Required

We are seeking an experienced and compassionate Case Manager to work with older adults and their families in a community-based social services program in Brooklyn.

The Case Manager will assess clients' social and emotional needs, develop individualized case plans, connect clients with community resources, provide advocacy, and coordinate services designed to help older adults maintain their independence and quality of life.

Key Responsibilities:

• Assess the social and emotional needs of older adult clients and their families

• Develop and implement individualized case plans

• Provide client and family counseling, guidance, and support

• Screen clients for benefits eligibility and assist with entitlement applications

• Advocate for clients and connect them with appropriate community resources

• Coordinate services including transportation, home care, Meals on Wheels, Medicare, Medicaid, housing, legal assistance, and other supportive services

• Evaluate clients' ability to manage their affairs and identify potential abuse or exploitation concerns

• Make appropriate referrals to Adult Protective Services and other community agencies

• Conduct ongoing reassessments and update case plans based on changing client needs

• Maintain accurate case records, reports, and required documentation

• Participate in community meetings, education groups, and agency trainings

• Travel to clients' homes as required

Qualifications:

• Bachelor’s Degree in Social Work required

• Bilingual Russian preferred

• Strong case management, documentation, and communication skills

• Comfortable working directly with older adults and their families

• Computer proficient and able to learn program-specific software

• Ability to travel to clients' homes using public transportation

• Compassionate, organized, dependable, and comfortable working independently in the community

Pay: $25–$26/hour
