Position Summary
The Case Manager / Addiction Counselor provides direct clinical care and case management services to clients in a residential treatment setting. This role manages an assigned caseload, delivers individual and group counseling, coordinates treatment planning and aftercare, and collaborates with the interdisciplinary team to support recovery from substance use and co-occurring mental health challenges.
This is a clinical role requiring sound judgment, strong counseling skills, and the ability to work effectively with diverse populations, including LGBTQIA+ individuals.
Core Responsibilities
- Provide individual counseling and facilitate group sessions, including process, psychoeducational, and life skills groups
- Conduct clinical assessments, including intake evaluations, bio-psychosocials, treatment plans, and discharge summaries
- Develop, implement, review, and update individualized treatment plans and monitor client progress
- Provide crisis intervention, advocacy, and de-escalation support as needed
- Manage an assigned caseload and coordinate care with therapists, medical staff, BHTs, and other clinical team members
- Coordinate discharge planning and aftercare services, including referrals to community resources
- Communicate with families, courts, probation officers, and collateral agencies as clinically appropriate
- Maintain accurate, timely, and compliant clinical documentation; document incidents per policy
- Participate in clinical meetings, case conferences, supervision, trainings, and in-services
- Perform other related duties as assigned, consistent with the scope of the role
Required Knowledge, Skills, and Abilities
- Strong understanding of substance use disorders, recovery principles, co-occurring mental health conditions, and 12-step programs
- Competence in individual and group counseling techniques
- Crisis intervention, behavior management, and de-escalation skills
- Excellent written and verbal communication skills
- Ability to maintain professional boundaries, confidentiality, and ethical standards
- Ability to work independently and as part of a multidisciplinary team
- Ability to remain calm and effective in high-pressure situations
- Demonstrated ability to work with diverse populations; LGBTQIA+ experience strongly preferred
Technical Skills
- Proficiency with electronic health records and documentation systems
- Basic computer skills, including professional email communication and document management
Minimum Qualifications
- Minimum of two years of experience working with individuals with substance use and co-occurring mental health challenges
- Background in behavioral health, counseling, social work, or a related field
Licensure, Certifications, and Required Training
- CADC-I or CADC-II certification required
- Completion of 16-hour IMS / medication training
- Training in UDS procedures, crisis intervention and de-escalation, and suicide risk awareness
- Demonstrated cultural competency
- Current CPR and First Aid certification
- Current TB test in compliance with facility requirements
